At Cobleskill, email is the official means of communication for the campus, so it is imperative that each student activate and use their Cobleskill network account and email service. In order to submit your medical history information, and immunization information, you will need to activate your Cobleskill email account. Click here and follow the directions. Once you have established your email account, you will then be able to proceed with the directions below. All future correspondence will be through your Cobleskill email.
Students who attend SUNY Cobleskill and are enrolled for 6 or more credits must submit complete medical history information, and immunization information.

Prior to arrival to campus, all students registered for 6 credit hours or more MUST submit the following:

1. Login to Patient Portal
Go to the Patient Portal and follow the directions to register. Once you have registered, you will receive a link in your SUNY Cobleskill e-mail account that will prompt you to create and verify your new password. This will give you access to your Patient Portal account where your medical history information and immunization information is submitted.
2. Submit Your Required Medical History Information
To complete and submit your medical history information, please select "Forms" and complete the "Medical History Form".
1. In the Patient Portal, click on "Forms"
2. Select "Medical History Form"
3. Fill out "Medical History Form"
3. Click on "Submit" (if the form does not disappear it means that you have missed a required question)
3. Submit Your Required Immunization Information
All immunizations must be verified by Wellness Center staff through one of the following methods.

* A copy of your immunization history may be obtained from your graduating high school or your family physician.
Your Health and Immunization Information will NOT be considered complete until a copy of your immunization record is received and verified by Wellness Center staff. Failure to submit your record, may delay your registration process.
NEW YORK STATE LAW: MMR REQUIREMENTS
Records may be located at your previous college, high school, or family doctor’s office.
In accordance with NYS Law any student born after January 1, 1957, must provide proof of immunity to measles, mumps, and rubella, this requirement may be met by the following: proof of disease, antibody titers, or two {2} measles, one {1} mumps, and one {1} rubella vaccine given after the student’s first birthday.
NEW YORK STATE LAW: MENINGOCOCCAL MENINGITIS VACCINE
The Center for Disease Control Advisory committee on Immunization Practices (ACIP) recently issued a new recommendation for college students registered for 6 credit hours or more: Students, especially those living in residence halls, should consider vaccination to reduce their risk for meningococcal disease.
New York State Public Health Law 2167 requires:
A response to receipt of meningococcal disease and vaccine information signed by the student or student’s parent or guardian (if under the age of 18)
AND EITHER
A record of at least 1 dose of Meningococcal ACWY vaccine within the past 5 years
OR
An acknowledgement of meningococcal disease risks and refusal of meningococcal vaccination response form signed by the student or student’s parent or guardian (if under the age of 18).
SUNY Cobleskill does not offer the meningococcal meningitis vaccine. Students should contact their private health care provider for vaccination. Click the following link for further information on Meningococcal Disease:

SUNY Student COVID-19 Vaccine Policy:
The SUNY student COVID-19 Vaccine Policy requires that students who have a physical presence on campus must provide evidence of the initial course of COVID vaccinations. Students are considered fully vaccinated for the initial course of vaccination (1) two weeks after receiving the second dose in a two dose COVID-19 vaccine series or (2) two weeks after receiving a single dose in a one dose COVID-19 vaccine series. All students are strongly encouraged to stay up to date on their vaccinations, including obtaining any booster authorized by the FDA and recommended by the Center for Disease Control and Prevention. While not requiring a booster at this time, SUNY reserves the authority to impose a requirement for any booster at any time as a result of changes in local conditions or as a result of requirements imposed by Federal, State, or local authorities.
Students may request an exemption from the COVID-19 vaccination requirement for medical or religious reasons. Exemption requests are evaluated on a case-by-case basis by a committee; they are not automatically granted. Outcomes of requests will be communicated through our secure messaging system. All students who are exempt must participate in the mandatory surveillance testing program. Students with exemptions may be excluded from all in-person classes and activities at the college’s discretion should a COVID-19 outbreak occur on campus. SUNY Cobleskill may impose additional restrictions as needed.
